Create a new case in the portal by completing a brief form and uploading the required scans and images.
Step 3: Custom Treatment Plan
Dr. Nirav Patel will create the treatment plan, and you will be notified when the 3D digital setup is ready for your review.
Step 4: Review & Approval
Once you are satisfied with the 3D digital setup and the overall treatment plan, submit your approval through the portal.
Step 5: Receive Aligners
Receive Smilewave aligners custom-designed for your patient and deliver treatment with ease.

To get started, visit portal.mysmilewave.com and click on “Request Portal Access.” Complete the required basic information. Once your request is reviewed, your account will be created and a portal invitation will be sent to you via email.
To initiate a case, the following must be submitted through the Smilewave portal along with a completed New Patient Form:
Required Images
- Frontal face (lips relaxed)
- Frontal face (smiling)
- Profile view (lips relaxed)
- Frontal intraoral (in occlusion)
- Right buccal (in occlusion)
- Left buccal (in occlusion)
- Upper occlusal
- Lower occlusal
Required Scans / Radiographs
- Panoramic radiograph
- Intraoral scan (STL files)
Once all required scans, X-rays, and patient information are received, the digital setup is designed and delivered within 3 to 5 business days.
The digital setup includes:
- A Smilewave Treatment Plan document
- A 3D digital setup viewer link, allowing you to review:
- Number of stages
- Planned tooth movements
- Predicted final outcome
- Zoom-in and zoom-out functionality for detailed evaluation
After approval of the digital setup, aligner production begins. The aligners are typically shipped within 10 to 14 business days.
Yes. The one-time price includes all necessary aligners and refinements required to complete treatment successfully.
Retainers are not included in the one-time treatment fee. However, they can be ordered through Smilewave at a minimal additional cost upon completion of treatment.
